MassHealth provides non-emergency transportation services as authorized by the client�s medical provider. This only applies to going to and from MassHealth provider for a MassHealth covered service.

To use this program, ask your medical provider to complete a PT-1 Form/Request for Transportation Services.

This form needs to be set to the MassHealth Transportation Unit for approval. If approved, you will receive written notification outlining how to arrange rides.� The program guideline is that it is for when public and/or private transportation is not otherwise available, although there are exceptions. Please call the number above for further information.
