MassHealth provides non-emergency transportation services as authorized by the client’s medical provider. This only applies to going to and from MassHealth provider for a MassHealth covered service.

To use this program, ask your medical provider to complete a PT-1 Form/Request for Transportation Services.

This form needs to be set to the MassHealth Transportation Unit for approval. If approved, you will receive written notification outlining how to arrange rides.· The program guideline is that it is for when public and/or private transportation is not otherwise available, although there are exceptions. They rely on volunteer drivers and can always use more. Anyone under the age of 18 must be accompanied an adult.
